Candace, It usually only takes one shot of baytril to “clean” up most infections. You do not want to remove any scutes unless they are shedding. If they are loose then they can be removed. Part of the scute can be removed that covers the infected area. You can get betadine and apply it topically to the local area only in a concentrated form. Allow it to dry before putting him back in water. I don’t think it is a good idea to do any anesthesia, there are some bad effects with most reptiles. I have worked with extensive shell rot and did not have to use anesthesia. Do not let the vet “de-worm” Tex. Internal parasites are not ususally a problem. Vets like to use ivermectin on most animals and this will kill turtles instantly. You do need to try and remove the fungus to prevent it from spreading. This will also speed the healing process considerably. You can soak in acriflavine or betadine dilutions instead of Nolvasan. Nolvasan is just a surgical scrub and the vet should have plenty of it if you wished to use it. You might can get it from a farm supply, COOP, or feed and tack type store as well.
